All Student Athletes are required by law to submit a completed Physical Exam Report, signed by their physician, prior to participation on a school team, at least once during the school season. Those students that participate in more than one sport will be provided with short form health history update which must be completed and returned to the school's athletic trainer, Karen Manista. In addition, students must submit an athletic permission slip, signed by their parents or guardians, and a signed policy consent form, which is consent to random Steroid testing.  The forms are provided below for Summit High School student athletes.

In addition, some student athletes, depending on the sports, will be required to complete a concussion baseline test prior to receiving clearance to participate in the sport.
