Read Carefully Part III-A- School Board Insurance Information
Complete Part III-B- Parental Insurance Information

Complete entire page and add all pertinent medical information
Page 3 � To be Completed by Medical Staff and Physician Orthopedic Examination Assessment
(All sections to be filled by medical staff)

Notarize prior to physical exam or
Leave blank and notarize the physical form the day of the physical exam. There will be a notary public present the day of the physicals.
Parent/guardian must be present and bring picutre ID

Insurance
All student-athletes must purchase student accident insurance before they can participate in:
Tryouts and off season conditioning
All sports insurance cost is $13.00
Fall football catastrophic insurance is $45.00. Football players must purchase both insurances.
All insurance will be collected the day of the physicals.

Do not participate in any physical activity 2 hours prior to the physical examination
If you wear glasses or contact lenses, bring them to the physical
All aspects of the physical examination are free
Wear comfortable shorts, shirt and tennis shoes
