Once you have opened the form, print it, fill it out, have it signed, and mail (with the $35.00 membership fee) to:

**** LIFE MEMBERSHIP ****

If you are applying for a Life Membership and this is your first year, please provde your coaching history to includes Dates (Years), # of Year(s) and name of  School/Organization of where you coached.  The document will require your signature and the signature of your current/past principal.  Please forward this documentation with your membership application.
